Utah Advanced Directive

The Utah Advance Directive allows Health Care Professionals give you the care you want when you cannot make decisions or communicate for yourself. It has four distinct parts.


PART I:

This allows you to name another person to make health care decisions for you when you cannot make decisions or speak for yourself.


PART II:

This allows you to record your wishes about health care in writing.


PART III:

This allows you to state how you want the directive revoked or changed.


PART IV:

This makes your directive legal.
